How to Enroll…
STEP 1:
Take a Tour
Tours must be scheduled in advance. We serve a vulnerable population and are doing everything we can to keep them safe.
Call Lea VanMerkestyn at 505-418-1300 or email her at director@lifecirclenm.org to schedule a visit. Please note that all clients are required to show proof of vaccination against COVID-19.

Start the Application Process…
Fill out the family Application and submit the Physician’s Form to your medical provider. You can download and print these yourself or pick up a paper copy at the Center. (If you have difficulty downloading the forms from the website, please email director@lifecirclenm.org and she can email the documents to you.)
